Mental Health Assessment and Referral Procedures – Emergent Care

EMERGENT CARE: Referral for HOSPITALIZATION of a student-athlete:


  1. A Risk Assessment needs to be completed either over the phone or face-to-face with the student-athlete.
    1. Ensure the student is in a safe place, is under control, and is not a risk to self or others.
    2. Risk Assessment needs to include an assessment for risk of suicide, including thoughts of death and/or suicide:
      1. An assessment of their risk of harm to others, their present safety, and if they are under the influence of alcohol or drugs.
      2. Along with questioning where the student-athlete is (an address/location), who they are with, and their phone number, along with a phone number of a person they are with.
      3. Plan, Intent , and Access to the Plan
  2. Following determination that a student-athlete is at risk of harm to self and/or a risk of harm to others, the following procedures are to be implemented for a Emergent Assessment:
    1. Contact Campus Police (preferable) at 217-333-1216 or 217-333-8911 (911) to perform a wellness check and to have the student-athlete transported by ambulance to the Presence Convenient Medical Center Emergency Room. When contacting Campus Police, ask to speak to a member of their Crisis Intervention Team (CIT) once you have made the request for a wellness check. Try to stay on the phone with the student- athlete, or stay with them if they are in your office/training room until the CIT Officer/ ambulance arrives.
    2. Request the student-athlete be transported to Presence Convenient Medical Center Emergency Room
    3. The Athletic Trainer or designated Division of Intercollegiate staff member will attempt to meet the Campus Police to introduce them to the student- athlete, and/or go to the Emergency Room to report information as needed and to support the student-athlete until they are admitted or discharged.If the student-athlete is not on campus (but still in Champaign-Urbana), contact the Campus Police, inform them they are a University student and a member of DIA and ask them to do a Wellness Check, or contact 911. Again requesting to speak to a member of the Crisis Intervention Team. You can also contact Randy Ballard to discuss the situation and to seek further guidance.
    4. Once the student is safe, contact Randy Ballard, Director of Sports Medicine.
      1. Following a discussion, it will be determined who will contact the Psychiatric Unit at Presence Convenient Medical Center in order to initiate an admission.
      2. Randy Ballard will contact Dr. Zimmerman, Dr. Henrichs, Dr. MacDougall, or Dr. Dumich along with any members of the Mental Health Team as deemed necessary.
        1. The designated staff member will contact Presence Psychiatric Unit floor at 217-337-2130 and speak to Cymi or Jackie, Unit Social Workers or if it is after 7:30 p.m. or the weekend, ask to speak to the Unit Nurse. They will contact the Doctor on-call for admission, and they will contact the ER regarding admission of the student-athlete.
        2. DIA staff or designated person will contact the Crisis Team/Assessment Request Line at Rosecrance at (217)531-3553 who will be assessing the student-athlete in the ER to give them your assessment and to share with them the information regarding the student-athlete.
          1. Give the following in a voice message:
            1. Leave your Name (who is calling).
            2. Identify the Company“University of Illinois Sports Medicine”.
            3. Who you are calling about and what ER the student-athlete is being transported to.And your phone number to receive a call back.
          2. DIA staff need to have the Crisis Line number put into their cell phones, so the number will automatically pop up when they call, but it is a good idea to leave their number in the voice message.
        3. If the student-athlete is not admitted and we have determined that this is in the best interest of the student-athlete, contact Randy Ballard and the Sports Medicine Physician who can come to the hospital and complete a Certification indicating the need for admission.
      3. Randy Ballard, or the Sports Medicine Physician, or the Athletic Trainer will contact the student-athletes’ parents/legal guardian following discussion and determination who is most appropriate to make this call.
      4. Randy Ballard will contact the Emergency Dean of Students Office at 217- 649-4129.
      5. Randy Ballard will inform Josh Whitman, Athletic Director for the University of Illinois as deemed appropriate and necessary.
  3. Following Admission
    1. Appropriate Releases of Information will need to be signed by the student-athlete for continuance of care and discharge planning at admission.
      1. Releases signed for parent/guardian.
      2. Releases signed for Sport Medicine Physician.
      3. Releases signed for Athletic Training staff, including Randy Ballard.
      4. Releases signed for Mental Health Staff including: Peggi White, James Warren, and Jim Maurer 
  4. If a student has admitted or expressed suicide, a Suicide Incident Referral Form (SIRF) must be completed per University policy.
    1. The form can be accessed on the Counseling Center website.
    2. If admitted the Psychiatric Unit at Presence will complete the form and notify the Emergency Dean.
    3. The student-athlete will be required to complete 4 counseling sessions with a Professionally Licensed Mental Health Professional per University policy. Completion of the 4 sessions must be reported to the Counseling Center once completed.If the 4 sessions are not completed, a “Hold” will be placed on the student- athletes account.
    4. The student-athlete will need to sign a Release of Information so the completion of the 4 sessions can be reported to the University of Illinois Counseling Center and Emergency Dean’s Office. The Release of Information will be completed by the Mental Health Provider who saw the student-athlete for the 4 sessions.
  5. If Presence Convenient Medical Center is not a viable option, have the student-athlete transported to The Pavilion, to Carle ER, or to another designated Psychiatric Mental Health facility.
  6. For an INVOLUNTARY ADMISSION:
    1. For The Pavilion call 217-373-1700 and ask for Assessment. We are to be sure to mention that this is a DIA student-athlete.
    2. The Athletic Trainer, Randy, or Sports Medicine Physician will ask to speak to the Admitting Psychiatrist on-call to give them history information re: the student-athlete and the present situation.
    3. Request that a Petition be completed by the Police Officer that has face-to- face contact with the student-athlete.Or, the Sports Medicine Physician can complete the Petition or Certification for an in-voluntary admission.
      1. The forms are available in the ER 

CRISIS CARE: Referral for ASSESSMENT of a student-athlete:


  1.   If a student-athlete is determined to be safe and is released following the assessment, the Athletic Trainer is to establish a check-in time to meet with the student-athlete face-to face within 24 hours. If the student-athlete is in need of Crisis Care, a referral to Peggi White, James Warren, Jim Maurer, or a member of the Sports Medicine staff is to be made for a Crisis Assessment and/or follow up. A referral and assessment by the Mental Health Professional or Sports Medicine will occur within 72 hours.
    1. Contact with Dr. Zimmerman, Dr. Henrich, Dr. MacDougall, or Dr. Dumich, and Randy Ballard, Director of Sports Medicine will be initiated to report a Crisis Care incident. 
    2. The Student- athletes's parents/legal guardian are to be contacted to inform them of the Crisis Care needed by their student-athlete.
      1. The parent/legal guardian’s aid will be initiated to check-in, supervise, and monitor the student-athlete, especially during the night and weekend.
      2. The student-athlete can also be required to come to the Athletic Training room at the football stadium or a Division of Intercollegiate building deemed appropriate by the Athletic Training staff for on-going supervision and check-ins.
    3. A Crisis Assessment with the Mental Health/Sports Medicine Team will be scheduled within 24 hours.
  2. If a student has expressed suicide, a Suicide Incident Referral Form (SIRF) must be completed per University policy. The form can be accessed on the Counseling Center website.
    1. Inform the student that they will be contacted by the University of Illinois Counseling Center.
    2. The student-athlete will be required to complete 4 counseling sessions with a Professionally Licensed Mental Health Professional per University policy. Completion of the 4 sessions must be reported to the Counseling Center.
      1. If the 4 sessions are not completed, a “Hold” will be placed on the student- athletes account.
      2. The student-athlete will need to sign a Release of Information so the completion of the 4 sessions can be reported to the University of Illinois Counseling Center. The Release of Information will be completed by the Mental Health Provider who saw the student-athlete for the 4 sessions.
